A 60-year-old man was hospitalized with fever and hypotension secondary to recurrent cellulitis of the left leg. He had a history of polysubstance abuse and hepatitis C. Elephantiasis nostras verrucosa was diagnosed based on bilateral nonpitting edema and hyperkeratotic verrucous lesions in the pretibial area. The patient's erythrocyte sedimentation rate and white blood cell count showed evidence of infection; osteomyelitis of the left fifth metatarsal head was suspected.

A 29-year-old sailor presented with a painless, enlarging, reddish, mobile lump under his left nipple. Four months earlier, while on deployment in the South Pacific, he had his nipples pierced on a whim.

abstract: Hemoptysis has many causes, including bronchiectasis, lung cancer, and bronchitis. The initial goals of the history and physical examination are to differentiate hemoptysis from epistaxis and hematemesis and then to establish its severity. A variety of signs and symptoms may suggest the underlying cause. For example, hematuria suggests vasculitis or an immunologically mediated disease, such as Wegener granulomatosis or systemic lupus erythematosus. The workup includes chest radiography and measurement of hemoglobin and hematocrit levels, platelet count, international normalized ratio, activated partial thromboplastin time, and creatinine level. Chest CT scanning often identifies sources of bleeding that are not apparent on radiographs and sometimes can be used in conjunction with bronchoscopy. Patients with massive hemoptysis should be hospitalized for rapid evaluation and intervention; treatment may include interventional bronchoscopy, angiography, or embolization. Structured treatment interruption (STI) before initiating a new antiretroviral regimen is one strategy for improving virological response in patients with multidrug-resistant HIV-1 infection. However, there are conflicting data concerning the effectiveness of STI, and its role has not been established. The results of several recent studies, described below, are noteworthy (Table).

Invasive fungal infections continue to be a major cause of morbidity and mortality in immunosuppressed patients. Two studies recently addressed the effectiveness of the newer antifungal posaconazole in the prevention of these infections. The first study compared posaconazole with fluconazole in patients with graft versus host disease. The second compared it with fluconazole or itraconazole in patients with prolonged neutropenia.

A recent study suggests a causal relationship between oral pathology-that is, gingivitis and periodontitis-and stroke. Periodontitis is the leading cause of tooth loss in the United States; the disease may affect nearly 50% of Americans older than 65 years. The pockets of infection that surround the teeth and penetrate into surrounding tissues function as chronic abscesses. The ongoing infection may seed the bloodstream with bacteria during flossing, toothbrushing, or professional cleaning.
